Building Pathways to Lives of Choice and Possibility
Supporting people to direct their own lives and build futures filled with purpose, belonging, and opportunity.
Every person deserves the opportunity to make choices, build meaningful relationships, contribute to their community, and pursue a life they value. People with intellectual and developmental disabilities have unique strengths, aspirations, and gifts that deserve to be recognized and supported throughout life.
The LifeCourse Nexus partners with developmental disability agencies, provider organizations, support coordination organizations, self-advocates, families, and community partners to strengthen person-centered systems through the nationally recognized Charting the LifeCourse framework. Together, we help organizations create lasting change that improves the lives of people and families.
Rather than introducing a new program or initiative, the Charting the LifeCourse (CtLC) framework enhances the work developmental disability organizations are already doing. By integrating person-centered approaches into service delivery, support coordination, life planning, and cross-system collaboration, CtLC helps organizations strengthen self-determination, community participation, and meaningful outcomes for people and families.
We help organizations integrate the Charting the LifeCourse® framework into existing roles, programs, services, and systems by strengthening capacity at four levels. Creating lasting change by strengthening people, professionals, organizations, and systems.
Building self-advocacy, planning skills, informed decision-making, and meaningful life planning through practical LifeCourse tools.
Strengthening person-centered practice through professional learning, leadership development, coaching, and Communities of Practice.
Building implementation capacity through strategic planning, quality improvement, organizational development, and sustainable person-centered practices.
Advancing collaboration across developmental disabilities, aging, behavioral health, Medicaid, housing, employment, education, transportation, and community partners.
Meaningful systems change begins with people.
Grounded in the Charting the LifeCourse framework, we help organizations strengthen leadership, align practice, build workforce capacity, and foster collaboration that improves experiences and outcomes for people, families, professionals, and communities.
Imagine communities where every person is recognized for their strengths, supported by meaningful relationships, connected to opportunities, and empowered to direct their own life. Let’s work together to make that vision possible.
