The LifeCourse Nexus partners with developmental disability agencies, provider organizations, support coordination organizations, self-advocates, families, and community partners to strengthen person-centered systems through the nationally recognized Charting the LifeCourse framework. How CtLC Strengthens Developmental Disability Systems

Rather than introducing a new program or initiative, the Charting the LifeCourse (CtLC) framework enhances the work developmental disability organizations are already doing. By integrating person-centered approaches into service delivery, support coordination, life planning, and cross-system collaboration, CtLC helps organizations strengthen self-determination, community participation, and meaningful outcomes for people and families.
