Rather than introducing another initiative, the Charting the LifeCourse (CtLC) framework enhances the work aging and disability organizations are already doing. By integrating person-centered approaches into long-term services and supports, care coordination, caregiver partnerships, and cross-system collaboration, CtLC helps organizations promote dignity, independence, meaningful community living, and better outcomes for people and families.
